Registering with us

Accountable / named GP for all patients

All patients registered at the Villa Street Medical Centre have a named doctor who has overall responsibility for your care and support. Your registered GP is also your named accountable and your allocated GP. Your allocated GP will be responsible for the overall provision of your healthcare. You are entirely free to see another doctor at the practice. 

If you are unsure of who is your named doctor please ask at reception.

All new patients are allocated a doctor at the point of registration.

Practice catchment area

The practice area is shown with our catchment area marked in red. Click on the map and enter your postcode to check if you live within our catchment area.

If you are unsure, you can also contact reception to confirm.

If you change your name and address or telephone number please inform us as soon as possible. If you move out of the practice area, you will be required to re-register with a more local practice.

How to register with us as a patient

If you live in our practice catchment area you can join our list.

It's easy to register with us at Villa Street Medical Centre:

  1. Complete a registration form which can be downloaded or are available from reception.
  2. Complete a GMS1 form which can be downloaded or are available from reception.
  3. Return your completed registration forms to reception along with your photo id (passport or valid UK drivers licence).

It is worth reading about our appointment system when choosing to register with us

If you are homeless or of no fixed abode, then although it is helpful to have photo ID you can register without any.

Any other ID such as an HC2 Certificate, Rough Sleepers' Identity badge, Hostel registration/mail forwarding letter would also be helpful but again we will register you even if you don't have these. It is more important that you can access health care when you need it.

Temporary residents

The doctors are able to see patients who are temporary residents for a maximum of 3 months in our practice catchment area should they need medical help. In this instance persons wishing to be treated should state that they are a temporary resident when reporting to reception.

To register you will need to bring your photo ID (passport or GB drivers license) and complete our Temporary Resident Registration form which is available from reception or can be downloaded here.
