Self Certification and Fit Notes (Med 3 form)

If you are requesting a fit note, also known as a Med 3 form and previously referred to as a sick note, do be aware that we can only issue them if you have been off work for 7 days in a row. This includes non-working days such as weekends and bank holidays.

For the first 7 days in a row off work you will need to provide your employer with a self certification certificate.

If this is the first time you are requesting a fit note then you will need to book an appointment with a GP so we can consult on the issue and plan your health care.

If you are requesting an extension to a fit note then we may be able to do this without a further appointment, instead our receptionists will ask some initial questions such as when your previous fit note ran out and how long you are requesting the fit note to be extended for. This information will help the GP when reviewing your fit note.

We don’t issue extensions to fit notes in advance of the original fit note expiring, however if you are concerned please call and speak to a receptionist who can advise.
